These Terms and Conditions shall be governed by and construed in accordance with the laws of the State of Texas, without regard to its conflict of law provisions. The exclusive venue for any proceedings will be the state and federal courts located in Collin County, Texas.

Simple Practice Client Portal and Telehealth Software and Service

The Simple Practice Client Portal Software and Telehealth Service is a solution provided to ensure privacy of all HIPPA allow information and communication, as well as for You and Your Provider at ARC to meet remotely using video, audio, and other media capabilities. SimplePractice Telehealth is part of the Service and allows You to attend Your appointments and connect with Your Provider for Telehealth consultations. The Simple Practice Platform is provided in accordance with our Privacy Policy, which can be found (HERE). As a user of this service, You agree to the use of your User Content and personal information in accordance with these Terms and ARC's Privacy Policy.

Account and Billing

Accounts must be current to receive service. A $300 fee applies to accounts not settled within 7 days of the due date, and service will be suspended on the 7th day of delinquency. Clients may place their membership on hold twice per calendar year for up to 30 days each.

Cancellation Policy

Program cancellations must be requested in writing 30 days prior to the effective date and will require an exit interview and completion of cancellation paperwork.
